Forms: All forms can be returned to the Registrar’s Office located in the Conlan Center, OR via your MSJ email to Registrar@msj.edu.
- Change of Major/Minor/Program Form: Complete the form; may obtain the necessary approval “signatures” via email.
- Add/Drop a Class Form: Complete the form with appropriate signatures.
- Incomplete Undergraduate Grade Petition Form: Complete the form with your instructor.
- Incomplete Graduate/Doctoral Grade Petition Form: Complete the form with your instructor.
- Learning Contract: Complete the form with your advisor.
- Petition to Review Completed Degree Requirements Form: For students whose graduation application has been canceled.
- Petition to Review for Certificate Posting Form: For students who are completing only a certificate, and not a degree.
- Waiver/Substitution for Course Substitutions Form: Complete the form; may obtain the necessary approval “signatures” via email.
- Request for Change in Personal Data Form: (addresses, phone, official name changes)
- Degree Requirements Exception Form Obtain: Complete the form; may obtain the necessary approval “signatures” via email.
- Non-Degree Enrollment: For students who are pursuing course work, but have not been accepted through the Admission Office and are not enrolled in a degree program at the Mount.
- Returning to the Mount after Withdrawal Form: For students who are returning to MSJ after being withdrawn for less then two years and who have not enrolled in any courses at another college or university while away from MSJ.
