Enrollment Steps and Forms

Here’s a list of all the forms you’ll need to enroll. Just start with Form 1 and complete the rest in order. You can submit forms online by clicking the "Online" links in the Access Online column. You can also download printable PDF’s of all forms by clicking the "Print" links in the Printable PDF column. 

You can complete all of your enrollment forms online except Forms 8, 9 and 10, which require a signature. When prompted, enter your username and password mailed to you by Admission. Please remember that your username and password are case sensitive.

Forms 1-6 must be submitted together to begin and ensure proper registration.

1  Deposit Allows you to register for classes and secure a room in the Residence Hall (if applicable). Online Print
2 Priority Registration Day Select a date to take COMPASS Math Placement Test and meet with an advisor to finalize your class schedule. Online Print
3 Declaration of Major Indicates your choice of major and alerts the respective academic department of your choice. Online Print
4 Course Scheduling Preferences Helps your advisor build a course schedule that accommodates your needs and time constraints. Online Print
5 Writing Self- Assessment Helps your advisor choose the best writing class for you. Online Print
6 Orientation Session Reservation Get details about requirements, options, and dates for orientation and welcome weekend event. Online Print
7 Resident Housing Application Provides your preferences for housing options in the Residence Hall. (This form is only required for resident students.) Online Print
8 Commuter Verification Indicates that you are a commuter student. This form is only required for commuter students. No Print
9 Medical History Provides critical medical information to Health Services.  No Print
10 Immunization Record Provides critical medical information to Health Services. (This form is only required for resident students.) No Print

Enrollment Checklist

  • I paid my deposit and submitted Form 1 - Tuition and Housing Deposit.
  • I completed and submitted Form 2 - Priority Registration Day.
  • I completed and submitted Form 3 - Declaration of Major.
  • I completed and submitted Form 4 - Course Scheduling Preferences.
  • I completed and submitted Form 5 - Writing Self-Assessment.
  • I chose an Orientation Session (1st and 2nd choice) and submitted Form 6 - Orientation Session Reservation.
  • I understand that the above six forms must be completed before I will receive a confirmation about scheduling classes.
  • I have completed either Form 7 - Housing Application or Form 8 - Commuter Verification.
  • If submitting Housing Application Form 7, I printed, completed and mailed Form 9 - Medical History and Form 10 - Immunization Record to The Wellness Center
